Provide direct patient care, including assessing medication needs, reviewing medication histories, and monitoring patient progress and response to drug therapy.
Collaboration:
Work closely with physicians, nurses, and other healthcare professionals to provide recommendations on optimal drug therapy.
Medication management:
Ensure medications are used safely and effectively by verifying prescriptions, determining correct dosages, checking for drug interactions, and managing medication programs.
Patient education:
Educate patients on how to use their medications properly, including dosage, usage, and potential side effects.
Record keeping:
Track patient data, including medication plans and progress, using electronic systems.
Strategic development:
Contribute to the development and implementation of clinical pharmacy programs.
Experience 1 to 3 years with Pharm D qualification is mandatory.
