Key Responsibilities
Review and interpret medication orders for accuracy, appropriateness, and potential drug interactions.
Provide clinical pharmacy services, including medication therapy management and patient counselling.
Monitor and evaluate patient responses to drug therapy and recommend necessary adjustments.
Collaborate with physicians, nurses, and other healthcare staff to optimize medication care plans.
Participate in clinical rounds and provide pharmacotherapy recommendations.
Maintain accurate medication records and ensure compliance with hospital policies and regulatory guidelines.
Conduct medication audits and support antimicrobial stewardship activities.
Educate patients on proper medication use, dosage, and side effects.
Assist in developing and implementing pharmacy protocols and clinical guidelines.
Provide training and guidance to pharmacy interns or junior staff when required.
