Conduct follow-up calls when required to ensure continuity of care.
Schedule appointments or connect callers with therapists, psychiatrists, social workers, or crisis intervention teams.
Coordinate with internal departments and external partners to ensure timely referrals.
Track and report service gaps or high-risk cases to supervisors.
