Job Summary The Home Care Health Nurse provides compassionate, high-quality professional nursing care to patients in their homes. This role involves assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining medical records. The nurse acts as a liaison between the patient, their family, and the physician to ensure continuity of care and improved patient outcomes. Key Responsibilities 1. Patient Care & Clinical Duties Assessment & Monitoring: Conduct comprehensive initial and ongoing physical and psychosocial assessments of patients in their home environment. Monitor vital signs and track changes in condition. Medication Management: Administer prescribed medications (oral, injection, IV) and treatments. Educate patients and caregivers on proper medication usage and side effects. Wound Care: Perform wound care, dressing changes, and decubitus care according to physician orders and infection control protocols. Specialized Treatments: Assist with medical equipment (e.g., ventilators, tracheostomies, feeding tubes, catheters) and monitor their functionality. 2. Care Planning & Education Care Plans: Collaborate with the interdisciplinary team and physicians to develop and update individualized Plans of Care (POC). Education: Teach patients and family members self-care techniques, disease management, and fall prevention strategies to promote independence. Discharge Planning: assist in the transition from hospital to home and eventually to discharge from home care services. 3. Documentation & Communication Record Keeping: Maintain accurate, detailed, and timely clinical notes and electronic medical records (EMR) in compliance with agency policies and regulatory standards. Collaboration: Report significant changes in the patient's condition to the physician and Clinical Manager immediately. Coordinate with physical therapists, occupational therapists, and home health aides.
